How do I get started?
First, and foremost, WELCOME! We are are so excited to start our journey together.
At Dynamic Kids Physical Therapy, Inc. we firmly believe that giving our patients a great experience is top priority. We want to ensure that you are making the most of your treatments during your session and throughout your daily activities.

What to do before your first session?
- Verify your out-of-network benefits using our Insurance Benefit Worksheet.
- Complete your patient intake forms including key past medical history, previous treatments, current medications, and other important information.
- Make a list of questions that you have to discuss with your therapist.
- If any specific objects to be used during the session were discussed please have them prepared before your first appointment.
- Optional: Prepare your child for the visit by discussing what may happen. For some children, by simply talking with them about what to expect, this may ease any anxiety they may have during the visit. For others it may only increase their anxiety. You know your child best.
What to expect on your first visit?
- You will complete any remaining registration paperwork.
- You and your child will meet with your physical therapist for their initial evaluation.
- During the evaluation your therapist will review questions about your child’s medical history and your concerns about their functional limitations.
- Your child’s movement, range of motion, strength, balance, coordination, general gross motor function, and much more will be evaluated.
- Your therapist will recommend and prepare a plan of care including duration and frequency.
- You will schedule a series of visits with your physical therapist. The amount of therapy needed will be discussed with you, depending on their recommendations.
- There may be treatments performed the first day to get you started.
- Your therapist will document their findings and treatment plan to submit to your referring physician, another medical professional, or for you to keep for your records.
- You will recieve a receipt to submit to your insurance company and/or to keep for your records.
- During the next visit you will review the activities with your child and progress accordingly per your plan of care.

Do You Take My Insurance?
We work with most major insurance plans on an out-of-network basis. Since we do not enter into contracts with insurance companies we are able to provide treatment that our patients need, rather than what their insurance plan dictates. The first step is to check with your insurance provider regarding your out-of-network benefits by completing our insurance benefits worksheet.
Out-of-Network
- For in-network or out-of-network clients.
- Provider does not have a contract with your insurance company.
- No surprise medical costs: Client pays for session on the day of service.
- Client submits claims to their insurance provider. Insurance company reimburses the patient.
- Quality versus Quantity: 1-1 Attention from therapist at a shorter duration (30 minutes).
- Reduced number of visits.
- No wait list! Faster access to services due to no insurance authorization or physician order required.
- Insurance companies cannot dictate the length, duration or type of services provided, including wellness services and intensive model = highly customizable to meet patient’s needs.
In-Network
- For in-network clients only.
- Provider is contracted with your insurance company.
- Insurance plan has set contracted payment rates, ever changing restrictions on what treatments qualify for reimbursement, and limitations for the amount of care a beneficiary is eligible for each year.
- Difficult for physical therapists to provide patients with high quality care that is specific to their individual needs.
- Client pays co-pay on the day of service.
- Company submits claims to client’s insurance provider. Patient pays the remaining balance to the insurance company.
- Quality versus Quantity: More time spent with Aides/Assistants at a longer duration (45 minutes to 1 hour)
- 2-3 visits per week

How Do I Submit A Claim To My Insurance Company?
It’s as easy as 1, 2, 3!
- After your visit Dynamic Kids Physical Therapy, Inc. will provide you with a receipt.
- Submit the receipt to your insurance company for reimbursement. The receipt has all of the necessary information: business name and address, tax ID, national provider identification, license numbers, patient’s ICD-10 code (diagnosis) and CPT (billing) codes.
- You may choose to submit bills following each visit, one time per month, or at any other interval, typically up to one year following your treatment visit.
