What to do before your first session

  • Verify your out-of-network benefits using our *Insurance Benefits Worksheet***Link here
  • Complete your **patient intake forms*** including key past medical history, previous treatments, current medications, and other important information.
  • Make a list of questions that you have to discuss with your therapist
  • Contact our office to discuss what to expect on your first session with your personal Physical Therapist
  • Optional: Prepare your child for the visit by discussing what may happen on their first visit (see “What to expect” section below). Note: For some children, by simply talking with them about what to expect, this may ease any anxiety they may have during the visit. For others it may only increase the anxiety. You know your child best 🙂
  • If any specific objects to be used during the session were discussed please have them prepared before your first appointment.

What to expect on your first visit

  • You will complete any remaining registration paperwork.
  • You and your child will meet with your physical therapist for the initial evaluation.
  • During the evaluation, your therapist will review question about your child’s medical history and your concerns about their functional limitations.
  • Your child’s movement, range of motion, strength, balance, coordination, and much more will be evaluated.
  • Your therapist will recommend and prepare a plan of care including duration and frequency.
  • You will schedule a series of visits with your physical therapist. The amount of therapy needed will be discussed with you, depending on the recommendations of your therapist.
  • There may be treatments performed the first day to get you started.
  • Your therapist will write up their findings and treatment plan to submit to your referring physician, another medical professional, or for you to keep for your records.
  • You will recieve a receipt within 24hours to submit to your insurance company or to keep for your records.
  • During the next visit you will review the activities with your child and progress accordingly per your plan of care.
